Back pain may feel like a dull ache, a sudden spasm, a sharp sensation, or pain that travels into the leg. Because several spinal structures can produce similar symptoms, diagnosing back pain often requires more than identifying where the discomfort occurs.

Why Different Back Problems Can Feel Similar
The back contains muscles, ligaments, discs, joints, bones, and nerves that work together to support movement. An injury or condition involving one structure may also affect surrounding areas.
For example, an irritated disc may cause pain primarily in the lower back. However, if that disc places pressure on a nerve root, the patient may also experience burning pain, tingling, numbness, or weakness in the leg.
Muscle pain can sometimes spread into the buttocks or upper thigh, while nerve-related pain may begin in the back and travel farther down the leg. Therefore, symptom location alone does not always provide a complete answer.
Doctors usually combine the patient’s medical history, physical examination, neurological testing, and imaging findings. No single test can independently identify every cause of back pain with complete accuracy.
How Doctors Begin Diagnosing Back Pain
The evaluation often begins with a detailed discussion about the symptoms.
A physician may ask:
- When did the pain begin?
- Did it start after an injury, lifting movement, or repetitive activity?
- Is the pain aching, burning, stabbing, or electric?
- Does it remain in the back or travel into the buttock or leg?
- Which positions or activities make it better or worse?
- Are numbness, tingling, weakness, or balance problems present?
- How does the pain affect sleep, work, walking, and other activities?
These answers help the physician recognize patterns. Pain that began after lifting and worsens when a particular muscle contracts may suggest a strain. Pain that changes with sitting, bending, coughing, or prolonged positioning may point toward a disc or another spinal structure.
Symptoms that follow a recognizable path into an arm or leg may indicate nerve irritation. However, these patterns provide clues rather than a final diagnosis.
How Muscle-Related Back Pain Is Evaluated
Muscle-related pain often develops after overuse, sudden movement, poor lifting mechanics, or an activity that exceeds the muscle’s current capacity. It may feel sore, tight, tender, or stiff.
During the physical examination, the physician may gently press different areas of the back to look for tenderness or muscle spasm. The patient may also be asked to bend, twist, stand, walk, or perform controlled movements.
Muscle-related pain may become more noticeable when the affected tissue contracts or stretches. In many cases, it remains concentrated in the back rather than traveling along a specific nerve pathway.
The doctor will also consider whether the discomfort could be coming from a spinal joint, ligament, sacroiliac joint, or another nearby structure. These conditions can sometimes produce symptoms that resemble a muscle strain.
Signs That Pain May Be Disc-Related
Spinal discs sit between the vertebrae and help absorb pressure during movement. A disc can deteriorate, develop a tear, bulge, or herniate. However, a disc abnormality visible on an MRI does not automatically mean that it is causing the patient’s symptoms.
Disc-related pain may feel deep and difficult to pinpoint. Depending on the location and condition, it may worsen with sitting, bending forward, lifting, coughing, or other activities that increase pressure around the spine.
When a disc irritates or compresses a nearby nerve root, symptoms may extend into the buttock, thigh, calf, or foot. The exact pattern depends on which nerve is affected.
An MRI can show discs, ligaments, nerve roots, and other soft tissues. Even so, doctors interpret imaging alongside the patient’s symptoms and examination findings rather than relying on the scan alone.
How Doctors Recognize Nerve-Related Back Pain
Nerve-related pain often has a different quality than ordinary soreness.
Patients may describe it as:
- Burning
- Shooting
- Electric
- Tingling
- Pins and needles
- Pain that travels down an arm or leg
A compressed or irritated spinal nerve may also cause numbness, reduced sensation, muscle weakness, or changes in reflexes.
During a neurological examination, the physician may test strength, sensation, reflexes, walking ability, and balance. Certain movements may gently place tension on a nerve to see whether they reproduce the patient’s familiar symptoms.
In selected cases, electromyography or nerve conduction testing may help evaluate communication between the nerves and muscles. These tests can provide additional information when symptoms, examination findings, and imaging do not present a clear answer.
Why Imaging Does Not Always Provide the Final Answer
X-rays can reveal fractures, alignment changes, arthritis, and other conditions involving bone. However, they do not clearly show discs, nerves, or most soft tissues.
MRI provides more detail about discs, ligaments, the spinal canal, and nerve roots. Nevertheless, imaging findings must match the patient’s symptoms.
A person may have a bulging disc or age-related spinal changes without experiencing pain. Another person may have significant symptoms even when the scan shows only subtle changes. This is why diagnosing back pain requires correlation between the examination, symptom pattern, and test results.
When several possible pain sources appear on an MRI, the physician may recommend a targeted diagnostic procedure to help identify which structure is most likely responsible.
What Is a Diagnostic Nerve Block?
A diagnostic nerve block is a targeted injection used to help determine whether a specific nerve or nerve branch is carrying pain signals from a suspected area.
The physician places a small amount of numbing medication near the selected nerve. Image guidance, such as fluoroscopy, may be used to help position the needle accurately.
The goal is not simply to provide long-term relief. Instead, the physician observes whether temporarily numbing that nerve changes the patient’s familiar pain.
A substantial temporary reduction in the usual symptoms may suggest that the targeted nerve is involved. If there is little or no meaningful change, the physician may consider another possible source.
Selective nerve root blocks can be helpful when symptoms and imaging do not clearly identify which nerve root is affected.
What Happens During a Diagnostic Nerve Block?
Before the procedure, the medical team reviews the patient’s health history, medications, allergies, imaging studies, and current symptoms. Patients should follow all instructions regarding eating, drinking, medications, and transportation.
During the procedure, the patient is positioned so the physician can reach the appropriate area of the spine. The skin is cleaned, and a local anesthetic may be used to reduce discomfort at the injection site.
Using image guidance, the physician advances a thin needle toward the targeted nerve. Contrast material may be used to confirm placement before the numbing medication is delivered.
Afterward, the patient may be asked to perform movements that normally trigger pain. Keeping track of how much the pain changes, how quickly relief begins, and how long it lasts can help the physician interpret the result.
Because the numbing medicine is temporary, any relief from a purely diagnostic injection may wear off within a limited period. The response provides information that may guide the next step rather than serving as a permanent treatment.
Does a Nerve Block Confirm the Diagnosis?
A nerve block can provide valuable information, but it is not interpreted in isolation.
The physician considers:
- Whether the injection reached the intended location
- How much the patient’s usual pain improved
- Which symptoms changed
- How long the response lasted
- Whether the result matched the examination and imaging findings
Pain is complex, and more than one structure may contribute to a patient’s symptoms. A positive response may support the suspected diagnosis, while an unclear response may lead to further evaluation.
Depending on the findings, the physician may recommend physical therapy, medication management, another targeted procedure, lifestyle changes, or a different treatment strategy.
Seek Immediate Care for Serious Symptoms
Most back pain does not require emergency treatment. However, patients should seek prompt medical attention for new loss of bladder or bowel control, numbness around the groin or inner thighs, rapidly progressing weakness, severe pain following significant trauma, or back pain accompanied by fever or other signs of serious illness.
These symptoms can indicate a condition that should not wait for a routine office appointment.
Determining whether pain originates from a muscle, disc, nerve, joint, or combination of structures requires careful evaluation. The process may include a medical history, physical examination, neurological testing, imaging, and, in selected cases, a diagnostic nerve block.
